That question stopped me in my tracks because it’s one I’ve heard dozens of times. Fatigue, weight gain, brain fog, hair thinning — and yet, their doctor says everything looks “normal.”
The missing piece? Thyroid antibodies.
These little troublemakers can start attacking your thyroid years before your TSH or T4 ever budge. Let’s break down what they are, why they matter, and how to understand your results like a pro.
Thyroid antibody tests look for signs that your immune system has gotten a little confused — and started attacking your thyroid tissue by mistake.
There are two main types worth knowing:
Thyroid Peroxidase Antibodies (TPOAb): Target an enzyme called thyroid peroxidase, which your thyroid uses to produce hormones. Elevated TPO antibodies are commonly associated with Hashimoto’s thyroiditis (autoimmune hypothyroidism) or Graves’ disease (autoimmune hyperthyroidism).
Thyroglobulin Antibodies (TgAb): Attack thyroglobulin, a protein your thyroid stores to make hormones later. TgAb can also indicate autoimmune thyroid activity and often shows up alongside TPOAb.

You can have normal TSH, Free T4, and Free T3 and still have elevated antibodies. That means your immune system has already begun targeting your thyroid, even if your hormone levels haven’t dropped yet.
This is why testing for TPO and TgAb is crucial:
