I once had a patient who said, “My cycles are all over the place — my doctor ran labs, and one came back as progesterone. What does that even mean?” That’s such a common question, because unlike estrogen or thyroid, progesterone doesn’t always get much attention until something feels off.
Progesterone is a powerhouse hormone, and the progesterone test can tell us a lot about fertility, cycle health, and even perimenopause and menopause transitions.
Progesterone is produced mainly in the ovaries after ovulation (and in smaller amounts by the adrenal glands). Its name literally means “pro-gestation” because it prepares the uterine lining for pregnancy.
The progesterone blood test measures how much of this hormone is circulating, usually taken mid-luteal phase (around day 21 of a typical 28-day cycle).
👉 Think of progesterone like the steadying force in your cycle — it’s the calm after the estrogen rise, helping balance mood, sleep, and reproductive health.
Progesterone does far more than support pregnancy:
Regulates cycles: Keeps periods consistent and predictable.
Balances estrogen: Prevents estrogen dominance symptoms like heavy bleeding or mood swings.
Supports sleep & mood: Calms the nervous system and promotes deeper rest.
Bone & heart health: Helps protect bone density and cardiovascular function.
Fertility & pregnancy: Prepares the uterine lining and maintains early pregnancy.
Low progesterone can show up as irregular or missed periods, spotting, PMS, anxiety, or sleep struggles. High progesterone is less common, but may be seen with pregnancy, ovarian cysts, or hormone therapy.
Reference ranges (women, ng/mL):
Follicular phase: <1.5
Luteal phase (after ovulation): 2 – 25
First trimester pregnancy: 10 – 44
For men: typically <1.0 ng/mL.

Progesterone levels rise and fall naturally with your cycle, but other influences include:
Stress: High cortisol can “steal” from progesterone pathways.
Thyroid function: Low thyroid can disrupt progesterone production.
Age & perimenopause: Levels often decline with aging.
Medications: Birth control, fertility treatments, and HRT.
Illness or adrenal dysfunction: Can alter hormone balance.
Lifestyle strategies can make a difference:
Balance stress: Meditation, breathwork, and rest lower cortisol’s impact on progesterone.
