I once had a patient look at their labs and say, “My doctor said my thyroid was fine — but I’m exhausted, cold, and gaining weight. How can that be?”

It’s one of the most common frustrations I hear. And often, the answer lies in a test that gets overlooked: Free T3.

Free T3 is your active thyroid hormone — it’s what actually gives your cells the signal to burn energy, regulate temperature, and keep your metabolism humming. If it’s too low, you can feel hypothyroid even when your TSH and Free T4 look “normal.”

Let’s break down what Free T3 really means, why it matters, and what might be getting in the way of optimal levels.

What Free T3 Measures

Free T3 (triiodothyronine) measures the amount of unbound, active thyroid hormone circulating in your bloodstream.

Your thyroid produces two main hormones: T4 (thyroxine) and T3 (triiodothyronine). T4 is the storage form — it’s not very active on its own. Your body has to convert it into T3 for it to actually do the work.

T4 is the paycheck — it’s the potential energy.
T3 is the cash — it’s what you can actually spend.

When your body struggles to convert T4 into T3, you can have plenty of “paychecks” (normal T4) but not enough “spending money” (active T3).

 

Why Free T3 Matters

Free T3 is the thyroid hormone that actually interacts with your cells — so it affects almost everything related to energy and metabolism.

When Free T3 levels are too low, you might experience:

  • Sluggish metabolism or difficulty losing weight

  • Fatigue or brain fog

  • Cold intolerance

  • Constipation

  • Hair loss or dry skin

  • Low mood or motivation

When Free T3 is too high, symptoms can include:

  • Rapid heartbeat or anxiety

  • Weight loss despite eating normally

  • Heat intolerance

  • Insomnia

T3 is powerful — it’s like the “spark plug” for your metabolism. That’s why small shifts can make a big difference in how you feel day to day.

 

Understanding Your Numbers

Typical reference range for Free T3 (pg/mL):

  • Normal: 2.3–4.2 pg/mL

⚠️ Note: Ranges vary slightly depending on the lab. Always review your results with your healthcare provider.

Optimal range: Many functional medicine practitioners consider the “sweet spot” to be in the upper third of the normal range — where symptoms often improve even if TSH is technically normal.

 

How Free T3, Free T4, and Reverse T3 Work Together

Your thyroid produces mostly T4, and your body converts that T4 into T3 as needed. But sometimes, stress or inflammation can send that conversion off course.

  • Free T4 → Free T3: The healthy conversion process that fuels metabolism and energy.

  • Free T4 → Reverse T3: When stress, illness, or inflammation is high, the body diverts T4 into Reverse T3, a “mirror image” hormone that blocks T3 from doing its job.

👉 Think of Reverse T3 like a decoy key — it fits the same lock but doesn’t open the door. It slows your metabolism when your body senses stress.

 

That’s why it’s so important to look at Free T3, Free T4, and Reverse T3 together — they tell you not just what your thyroid is producing, but how well it’s being used.

 

Factors That Affect Free T3

Several things can influence how much Free T3 your body produces and uses:

  • Chronic stress or high cortisol: Pushes conversion toward Reverse T3.

  • Low-calorie or restrictive diets: Signal the body to conserve energy.

  • Inflammation or illness: Suppresses conversion from T4 to T3.

  • Nutrient deficiencies: Selenium, zinc, and iron are critical for healthy thyroid hormone conversion.

  • Liver or gut health: Conversion primarily happens in these organs.

  • Medications: Beta blockers, steroids, and some antidepressants can affect T3 levels.
