Understanding Bilirubin: What Your Liver Is Trying to Tell You

I’ll never forget a patient who came in worried because her lab results said her bilirubin was “high.” She looked fine, felt fine, and was convinced she was dying.
Spoiler: she wasn’t.
Her liver was just sending up a little flare.

 

Bilirubin is one of those labs that shows up on your routine blood work but doesn’t get much explanation — unless it’s wildly off. Yet it’s one of the best quiet indicators of how your body is processing waste and handling detoxification.

 

So let’s break down what bilirubin actually means, why it matters, and what your numbers might be trying to tell you.

What Bilirubin Measures

Bilirubin is a yellowish pigment made when your body breaks down old red blood cells. Those red cells live for about 120 days, then your liver steps in to recycle their parts — including hemoglobin, which turns into bilirubin.

 

Your liver then processes (or “conjugates”) that bilirubin and helps your body excrete it through bile. In other words:
🩸 Your blood breaks it down.
💛 Your liver cleans it up.
🚽 Your body flushes it out.

 

That’s why bilirubin is such a useful measure of liver function — it shows how well your liver is doing its cleanup job.

 

The Three Types of Bilirubin (and Why They Matter)

Your lab report may show:

  • Total Bilirubin: The combined amount of direct and indirect bilirubin in your blood. This is the main number to watch.

  • Direct (Conjugated) Bilirubin: Bilirubin that’s been processed by the liver and is ready to be excreted.

  • Indirect (Unconjugated) Bilirubin: Bilirubin that hasn’t yet been processed by the liver.

If total bilirubin is elevated, your provider may look at the direct and indirect breakdown to see where the problem lies — whether your liver is struggling to process bilirubin, or your body is producing more than it can handle (for example, from red blood cell breakdown or stress on the liver).

 

Why Bilirubin Matters

Think of bilirubin as your liver’s report card. It reflects how efficiently your liver is breaking down and clearing waste.

 

When bilirubin builds up, it can cause yellowing of the skin or eyes (jaundice) — but even smaller elevations can point to subtle issues like:

  • Liver inflammation or sluggish detox pathways

  • Gallbladder or bile flow issues

  • Hemolysis (breakdown of red blood cells)

  • Hormone-related sluggishness (especially estrogen dominance)

  • Certain medications or fasting

Low bilirubin, on the other hand, is rare and usually not a major concern — but it can appear with oxidative stress, certain infections, or nutrient deficiencies.

  • Alcohol: Even moderate use can increase bilirubin levels over time.

  • Genetics: Conditions like Gilbert’s Syndrome (a benign liver enzyme variation) can cause mild bilirubin elevations.

 

Supporting Healthy Bilirubin Levels

Healthy bilirubin levels reflect a healthy liver — and supporting your liver doesn’t require a detox smoothie cleanse.


Here’s what actually helps:

  • Stay hydrated. Water supports bile flow and waste removal.

  • Eat colorful fruits and veggies. Antioxidants like vitamin C and glutathione precursors (found in broccoli, spinach, and avocado) help your liver process bilirubin.

  • Support gut health. Your gut helps eliminate bilirubin; constipation can lead to reabsorption.

  • Limit alcohol and ultra-processed foods. Give your liver less to filter.

  • Get enough protein. Amino acids like glycine and cysteine are key for detox pathways.

  • Manage stress. Chronic stress affects cortisol, which can slow liver metabolism and detoxification.
