I’ll never forget a patient who came in worried because her lab results said her bilirubin was “high.” She looked fine, felt fine, and was convinced she was dying.
Spoiler: she wasn’t.
Her liver was just sending up a little flare.
Bilirubin is one of those labs that shows up on your routine blood work but doesn’t get much explanation — unless it’s wildly off. Yet it’s one of the best quiet indicators of how your body is processing waste and handling detoxification.
So let’s break down what bilirubin actually means, why it matters, and what your numbers might be trying to tell you.
Bilirubin is a yellowish pigment made when your body breaks down old red blood cells. Those red cells live for about 120 days, then your liver steps in to recycle their parts — including hemoglobin, which turns into bilirubin.
Your liver then processes (or “conjugates”) that bilirubin and helps your body excrete it through bile. In other words:

🚽 Your body flushes it out.
That’s why bilirubin is such a useful measure of liver function — it shows how well your liver is doing its cleanup job.

When bilirubin builds up, it can cause yellowing of the skin or eyes (jaundice) — but even smaller elevations can point to subtle issues like:
